Joining Nuestra Familia Medical Group simply starts with your health plan. Nuestra Familia Medical Group has contracts with the leading health plans which include HMO (health maintenance organization), POS (point of service), and PPO (preferred provider organization) plans.
Step 1
After you have chosen Nuestra Familia Medical Group as your medical group, you are asked to choose your primary care physician (PCP). Simply click on the Find a Doctor link to see a list of Nuestra Familia Medical Group physicians.
Step 2Once you have selected your preferred doctor, call and inform your health plan of the name of the physician you have chosen and make sure to tell them the group you are selecting is Nuestra Familia Medical Group. Your health plan will direct you from there.

- Direct Referral by your physician to the most commonly used specialists so you don't have to wait for an authorization
- We provide tools to our physicians to identify those individuals that have not yet obtained the recommended screenings for preventative health, such as immunizations, mammography, colorectal screenings, diabetes and cholesterol management so you and your family stay healthy
- Customer Service Representatives dedicated to assisting you with any healthcare issues you may have including appointment scheduling for your convenience
- Care Plus Program for those members requiring hospitalization to ensure that once you leave the hospital you are on a fast track to wellness
- Health and Wellness Classes to assist you in controlling your diabetes, coping with stress, weight management for children and adults, just to name a few
- Transportation Coordination for those members who do not have an ability to obtain their healthcare services as result of transportation limitations
